Introducing the CHHC On-Site Medication Dispensary
The Community of Hope Health Clinic is a member of the Dispensary of Hope based out of Nashville, Tennessee. Each year, CHHC is able to provide medications for CHHC patients free of charge for being a part of this program. The CHHC On-Site Medication Dispensary is a closed site and therefore only available for established patients who use CHHC as their primary care provider. Per CHHC policy, CHHC does not prescribe or dispense controlled substances or other pain medications.
Medications
Please take your medication samples as instructed by your medical provider. If you have any questions about to take your medications, please contact your Clinic Coordinator.
CHHC Dispensary Hours
Your medication may be picked up during the following times:
Monday-Friday: 9 am-2 pm
Thursday: 4-8 pm
Refills Requests
YOU MUST REQUEST YOUR REFILL IN WRITING AT LEAST TWO (2) WEEKS IN ADVANCE DUE TO THE CLINIC'S LIMITED SCHEDULE.
External Pharmacies
We realize that some of our patients will need to continue to have their prescriptions filled at commercial pharmacies such as Walmart, Publix, CVS, Walgreen's, etc.
If you are an established patient and need refills at an external pharmacy or have questions about your medications, you can email refills@communityofhopeclinic.com. We still ask that you ask your pharmacy to fax a request to (205) 574-3117.
In the subject line, please put your full name, date of birth and who your provider is. In the email, please include which medications you wish to have refilled, Per clinic policy, we ask that you or your pharmacy give us notice of refills 14 days PRIOR TO YOU RUNNING OUT OF YOUR MEDICATION..
