PATIENTS

The Community of Hope Health Clinic (CHHC) is now seeing patients by appointment only on Mondays and Tuesdays, and Thursday evenings. We do not see walk-ins.  Intake for new patients is held in our office on  Thursdays between 10 a.m.-12 p.m. (see application below)  The interviewing process to be seen in the clinic for new patients lasts approximately 15 minutes. To be eligible:

  • You must live in Shelby County, AL and provide proof of this, such as a utility bill or any other form (notarized) that states you live where you say you live

  • You must not have any health insurance or government plan including Medicaid, Medicare, or VA Benefits

  • You must be between the ages of 19 and 64 (unless you are not eligible for Medicare)

  • You must be at or below 200% of the poverty level based on total household gross monthly income as determined by government standards

  • Income verification can be a paystub, and award letter for Disability or Unemployment Compensation, or a letter from the person who might be supporting you.  If you are on Food Stamps you meet our financial guidelines.

 

If you are eligible for our clinic, you will be given an appointment for the Monday or Tuesday day clinic, or the Thursday night clinic. Day clinic appointments have specific times from 8 a.m.-12 p.m..  Thursday night clinic starts at 5:00 pm and you must be present by 4:30 pm unless you notify us that you will be late.

Note:

  • As a rule our physicians do not prescribe narcotic pain medications.

  • We do not treat any Gynecological issues.  You can go to the Health Department or we can provide a resource list.

  • We do not provide psychiatric services. We usually refer to Chilton/Shelby Mental Health.
